Home & Community-Based Services

The HCS (Home and Community-Based Services) Program is a Medicaid waiver program that provides individualized services and supports to Texans with intellectual disabilities or related conditions. These services help individuals live in and participate in their communities.

As a contracted provider with the Texas Health and Human Services Commission (HHSC), Harmony Living Home delivers comprehensive services that supplement other community programs, ensuring each individual receives the specific support they need to thrive.

Behavioral Support

Supported Employment

Assistance finding and sustaining meaningful paid employment opportunities in the community.

Respite Care

Short-term relief services for unpaid caregivers, ensuring continuity of care while giving families breaks.

Physical Therapy

Skilled treatment to improve range of motion, physical conditioning, and mobility for daily independence.

Occupational Therapy

Skilled treatment designed to help individuals achieve independent functioning in daily living skills.

Speech Therapy

Corrective and rehabilitative treatment for individuals with speech, hearing, or language disorders.

Dietary Services

Licensed dietician assessments, personalized nutrition plans, and staff training for proper dietary management.

Adaptive Aids & Home Modifications

Eligibility

Who Can Receive HCS Services?

The HCS Program serves individuals with intellectual and developmental disabilities who meet certain criteria. Eligible participants can choose their service provider and may change providers at any time.

Diagnosis of intellectual disability or related condition
Mild to severe deficits in adaptive behavior
Eligible for Medicaid benefits
Not enrolled in another Medicaid waiver program
Desire to live in and participate in the community
